The NHS Pathway
How it works
- Speak to your GP — Tell them you're interested in GLP-1 treatment for weight management. They'll check your BMI, discuss your health history, and assess whether you meet the basic criteria.
- Referral to specialist service — If appropriate, your GP refers you to a specialist weight management service. Under NICE TA875, Wegovy must be initiated through these specialist services, not directly by GPs.
- Specialist assessment — The specialist team assesses your full eligibility: BMI threshold, weight-related conditions, previous weight management attempts, and suitability for treatment.
- Treatment begins — If eligible, Wegovy is prescribed and dispensed. You'll have regular follow-up appointments with the specialist team.
- 6-month review — Treatment is reviewed after 6 months. You must have achieved at least 5% weight loss to continue. Maximum treatment duration is 2 years.

The Private Clinic Pathway
How it works

- Prescriber review — A GMC-registered prescriber reviews your submission. They may ask follow-up questions or request blood test results.
- Prescription issued — If clinically appropriate, the prescriber issues a prescription for Wegovy or Mounjaro. If not suitable, you're told why and aren't charged.
- Medication delivered — Dispensed by a GPhC-regulated pharmacy and delivered to your door, typically within 2-3 working days.

The Postcode Lottery
NHS access to GLP-1 treatments varies significantly across England. Each Integrated Care System (ICS) commissions its own specialist weight management services, and provision is uneven.

- Some ICS areas have paused new referrals due to capacity or funding constraints
- The situation is changing — NHS England is expanding access, but progress varies

Which Route Is Right for You?
NHS may be best if...
- You meet NICE criteria (BMI 35+, or 32.5+ with comorbidity)
- Cost is a significant factor
- You have a specialist weight management service with reasonable waiting times in your area
- You want the multi-disciplinary support (dietitian, psychologist) that specialist services provide
- You're happy to start with Wegovy (the only NICE-approved option)
Private may be best if...
- You want to start treatment quickly (no waiting list)
- Your BMI is between 27-35 (below NHS threshold but eligible privately)
- You want the option to try Mounjaro (not available on NHS for weight loss)
- There's no specialist service or long waiting lists in your area
- You can afford the monthly cost
You can do both
Many people start with a private clinic for immediate access while asking their GP about an NHS referral. If NHS treatment becomes available, you can transition. Just make sure both your GP and private prescriber know about all medications you're taking.
